PCP need to recognize, know when to refer and co-manage chronic kidney disease with nephrologists.
The team approach works best.
The best treatment of CKD is accomplished with a multi-disciplinary approach built around a close collaboration with nephrologists, but frequently requiring additional input of care from diabetologists, cardiologists, and other specialists. The authors recommend that patients be referred for nephrology evaluation when GFR ranges between 60-30 Once GFR falls below 30, patients should receive regular nephrology follow-up.

The SHEA [Society of Healthcare Epidemiology in America] position paper summarizes a comprehensive review of the literature on UTIs in long-term care facilities. While acknowledging the high prevalence of bacteriuria and the difficulties of diagnosing and treating UTIs in this population, this article urges caution in attributing symptoms to UTIs and restraint in using antibiotics to treat bacteriuria.

Follow Up of Abnormal Clinical and Imaging Findings of the Breast: Five Self-Study Modules for Primary Care Clinicians
New from the CDC -- 5 self-directed, interactive training modules, designed to enhance clinical skills in the examination of the breast. This curriculum reviews the latest on the presentation and management of common breast problems, with an emphasis on assessment and early diagnosis. CEUs and CMEs available.
All of the modules are recommended for physicians. Registered nurses would most likely benefit from the first module on breast anatomy and the final module on diagnostic follow-up. The second module can be filed for reference by nurses to assist them in interpreting test results for purposes of patient/family/staff education.

A 93 Year Old Man with Advanced Dementia and Eating Problems
This article weaves a case study dilemma and covers the latest scientific information on g-tube decision-making in patients with advanced dementia (or advanced cancer).
Brings up questions to consider such as:
Will patients suffer more if they do not receive IV fluids or sufficient nutrition?
Are there advantages of conscientious hand feeding and challenges for staff?
Does tube feeding prolong life? Improve nutrition? Prevent aspiration?
Most physicians/providers automatically say yes. However, there is a paucity of evidence to support these purported benefits.
Of course, researchers cannot do the appropriate randomized study to answer these questions. The best available cohort studies suggest that tube feeding does not prolong survival, and has not been shown to improve nutritional status or the clinical consequences of malnutrition, such as pressure ulcers in the long term. This is clearly the case with dementia, and holds true to some degree with cancer and HIV/AIDS. The authors emphasize that feeding decisions should follow an ethical framework reflecting the balance of benefits/burdens/respect for autonomy. The authors offer general steps or an approach for clinicians to operationalize this framework.
